Over the course of my career, I’ve had the privilege of caring for you and your families. It’s been such an honor to be your physician and help you manage your healthcare.

This next phase of my medical career is going to be focused on providing services for a smaller group of patients, working in partnership to further optimize their health and well-being. An important step in this process will be the development of a personalized wellness plan.

We’ll discuss and address results of your examination and tests, identify areas on which we need to focus, and craft a wellness plan designed to help you maintain your health as well as manage any acute and/or chronic illnesses.

Membership

The annual cost for this program is $1,909 per patient, with discounts available for additional family members. Download the information packet or pick up the membership forms during your scheduled office visit.

If you’re not ready to join

If you choose not to become a member of the program, we’ll provide referrals so you can establish care with a different physician.
