Over the course of our careers, we have had the privilege of caring for many of you and your families. It has been such an honor to be your providers and share in your quest for a healthy, active life.
As you know from personal experience, our healthcare system is becoming increasingly complex. From our perspective, the many changes underway in healthcare will continue to affect our relationship with you and our ability to provide the highest quality care. After much reflection and soul-searching, we have decided that we do not want to compromise our diligent attention to your individual care.
Therefore, we are going to devote the next phase of our medical careers to providing services for a smaller group of patients. We will work together, in partnership, to further optimize your well-being. An important step in this process will be the development of your personalized wellness plan. We will meet with you to discuss the results of your examination and tests, identify the areas on which we need to focus, and craft a wellness plan personalized just for you. Together, we will ensure that your plan is implemented and adjusted as needed. We will continue to bill your insurance company for the delivery of your medical services.

The annual cost for this program is $2,500 per patient, with discounts available for additional family members. Download the information packet or pick up the membership forms during your scheduled office visit.
If you choose not to become a member of the program, we’ll provide referrals so you can establish care with a different physician.
