Dr. Claire imho seemed only interested in treating upper $$ middle class Signature patients and not Medicare patients. I terribly miss Dr. Leo Rogers, retired. Dr. Leo Rogers’ is the greatest doctor I’ve ever had in my entire life.

Medicine's rapidly changing landscape & increasingly higher costs plus the inevitable changes of age drove me to find a new generalist physician who would listen to me & see & treat me as a whole person. After meeting & discarding 2 other recommended physicians, I struck gold with Dr. Palley. I'm a retired MD myself. I know better than most how complex medicine has become; that earlier treatment, wider options & complex care can greatly improve outcomes & longevity. I get it. But the intelligent, broadly-trained, generalist whose wide clinical experience brings the ability to see the patient as a whole person has been lost along the way. When large corporations buy medical practices & run them as a business, their goal is to sell health care services to consumers to extract profit. Now patients are merely customers to be sold a test or procedure, and visits with a specialist for every symptom they report. Thus a woman with a long standing history of common migraine which hasn't changed in years--a problem any well trained GP or physician mid-level could handle -- is referred to the corporate neurologist who orders an unnecessary cranial MR at the corporation imaging center, to be read by their employed radiologist and so on. All a giant waste of time, energy & money with no noticeable improvement for the patient's life. While specialist knowledge is definitely the key to many improvements in health care, that's only true once a definitive diagnosis is reached. And therein lies the problem: modern-day corporate medicine can't deliver even adequate health care to patients who present with multiple health problems & needs at the same time, because it doesn't see the patient as a "whole“. There are no generalists to hear how the patient's life is disrupted, which often guides therapeutic decision making. Instead, the “generalist“ at today's corporate-owned practice is there to identify which body system might be responsible for the patient's misery then direct them to the specialists who deal with that system. It's become a common experience for the patient with several things bothering them to be “assessed" by a mid-level then referred to multiple specialists. Over the next weeks, special tests are done to rule out serious things that might be going on (defensive medicine is expensive) & after a follow-up visit, each specialist reassures the patient none of those rare, terrible things is present. The corporate bottom line is greatly enhanced, large expenditures made on administrative costs, but the patient is no better off. Weeks to months later, the patient is still miserable or getting worse & no one knows what the answer is because no one has listened to the patient. This is what happens when diverse symptoms arising from as-yet - unknown causes are parted out & evaluated from the get-go by specialists. Specialists aren't special; they just know a great deal about one small area. They are lost when presented with several problems interacting simultaneously, but this is where a well trained & experienced generalist shines. There are many different ways to deliver health care & it seems they change constantly. The model that works well in one situation or at one time of life may not work well in other situations. A model that works best for providers may not be the best model for users. Health care plans often boast how deep their provider panel is. While having a choice of providers is always good, patients need to remember that having different models is just as important, and that high quality physicians are picky; they choose the best models. Maybe other health care models end up with deep panels for a reason.
